Partners In Care
The Physician Alliance formed a 50/50 partnership in 2011 with St. John Providence Health System that created SJP Partners In Care. This partnership provides an innovative approach to delivering coordinated care to patient populations and enables TPA physician members to participate in a variety of health plans and program. SJP Partners In Care is the contracting entity for managed care and other health plan contracts. More than 232,000 managed lives are included in these programs.
Partners in Care employs a Practice Resource Team (PRT) that consists of registered nurses and practice transformation partners who work closely with The Physician Alliance staff and its member practices. A PRT member is assigned to a physician practice to provide support and expertise related to: transformation processes needed to become a BCBSM Patient Centered Medical Home or a Patient Centered Medical Home Neighborhood; navigating requirements for a practice to be successful in pay for performance programs; and guidance to improve quality, cost and efficiencies in patient care. The PRT member strives to be a key resource to the practice team and is committed to helping each practice reach their maximum potential in patient care and earned income.
