Diabetes Management Toolkit

Diabetes Management Toolkit

With diabetes being a complex disease requiring self-management, it’s important for practices to educate and provide resources to patients to help them manage their disease and to reduce risks for diabetes complications. According to The Centers for Disease Control and Prevention (CDC):

  • More than 2 in 5 U.S. adults have prediabetes
  • Prediabetes is a major risk factor for type 2 diabetes

The Physician Alliance created the diabetes management toolkit to assist our physician members and their practices to help give patients knowledge and skills to manage their diabetes. Through outreach efforts, practices can follow up with patients to get needed health screenings booked. EMRs and patient portals can also be used to send reminders and provide resources on improving patient health.
