The Physician Alliance is hosting a care management workshop through Michigan Institute for Care Management and Transformation (MICMT). The Introduction to Team-Based Care course helps participants better understand how to work in a multidisciplinary care team and in collaboration with the patient.

The course is required to bill care management/care coordination codes. Open to all members of the practice to gain foundational knowledge in team-based care, the course includes:

• Why, What, Who and How: Team-Based Care
• Care Management Process
• Outcomes and Triple AIM
• Billing Applications

Training Cost: This training is FREE to The Physician Alliance (TPA) primary care practices participating in BCBSM Physician Group Incentive Program (PGIP).
