Practice Transformation

The  Physician Alliance’s practice transformation team works closely with member practices to maintain and improve high quality of care for patients.

The practice transformation team is comprised of The Physician Alliance associates and the practice resource team (PRT). This team works 1:1 with member practices to ensure high quality of patient care and success in pay for performance initiatives.

The PRT consists of registered nurses and practice transformation partners who provide support and expertise to primary care practices to implement new PCMH capabilities and specialty practices that engage in PCMH-Neighborhood capabilities. The team also works with offices to roll out new practice standards, write processes and procedures, provide office units with educational material, and act as a liaison between The Physician Alliance and practice unit. The team works in provider offices to help close gaps in care and assists in patient outreach methods.

Each year, every practice participating in the PCMH program is eligible to be selected for a Blue Cross and Blue Shield of Michigan audit. The team works closely with those practices to ensure the practice is prepared for the audit visit.

This unique team provides face-to-face support, as well as constant virtual communication, to practices. With access to an exclusive, members-only portal, TPA offices have round-the-clock access to gaps in care, progress reports, designation dashboards, and numerous other valuable reports.
