Community Support

According to the Office of Disease Prevention and Health Promotion- Healthy People 2030, social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. The five domains of SDOH are economic stability, education access and quality, health care access and quality, neighborhood and built environment, and social and community context.

 The Physician Alliance has a certified community health worker on staff to assist PGIP member practices in supporting patient social determinants of health needs. TPA’s community health worker is available to work with patients to address their needs and concerns that are impacting their health and can help with accessing available resources. This is a complimentary TPA service to member practices.

PGIP physician member practices can access a community health referral form for a patient through TPA’s portal at www.tpareporting.org. Once logged into the portal, click “SDOH Referral” on the left side menu.
