According to Healthy People 2030, social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. The five domains of SDOH are economic stability, education access and quality, health care access and quality, neighborhood and built environment, and social and community context.
The Physician Alliance recommends FindHelp.org, for community referrals for our physician offices to address their patients’ SDOH needs. FindHelp.org can be used to connect people to community resources whether they are trying to stay well, meet basic needs, manage illness or care for others. This service is free to TPA members.
