Specialist Team-Based Care

The Specialist Team-Based Care value-based reimbursement (VBR) opportunity is available to five specialties:

• Cardiology
• Endocrinology
• Nephrology
• Oncology
• Pulmonology

 Participating practices are eligible for the 105% VBR if specific program criteria are met. Since specialists play such an important role in the care of patients who are likely to be admitted, it is vital to engage them in care that can prevent hospitalizations. Having care management available to improve care coordination is the premise of Specialist Team-Based Care initiative. In addition, receiving admissions, discharge, and transition (ADT) notifications via a mobile device is required to coordinate care for high-risk patients.
